Here is a (mostly complete) opening from a forgotten pilot, Two Guys From Muck. (The audio doesn't come in until about four seconds into this clip.) The credits (with pertinent shots of the actors involved) are as follows:

This is famously the pilot that then-NBC network president, Fred Silverman, walked out of a screening for (presumably because it was so awful - an interesting irony in and of itself, given the amount of flops aired on the network during his reign as president).

Also includes a partial commercial for Colgate Winter-Fresh Gel (the first ten or so seconds before the recording cuts out) - (C) 1981, Colgate-Palmolive Company

This show was created in 1979, only to be burned off in 1982. NBC dumped it in the time slot against the Oscars - virtually assuring that almost no one saw it. I think this may have accidentally been recorded by someone desperately trying to find the channel changer.

This aired on local Chicago TV on Monday, March 29th 1982 during the 9pm to 10pm (an hour-long sitcom? What the Muck?).
